Well, it may have something to do with sexuality. You never hear of Captain America's wife ... which makes you wonder. And you know how the military is with that sort of thing.

Why was Captain America never promoted to general? This sounds like a bad joke, but "Captain America" (the Marvel Comics hero dressed in red, white, and blue with the shield) is a code name the U.S. used for its first super-agent or "super soldier" during World War II. There's no official rank attached to it.

He had a rank in the US military before they gave him the Super Soldier serum. After that Captain America became his super hero name, not a rank in the military.
