Does it mean that all talk up to that point has been less than honest?

Yes and No... as the speaker may have a bad habit of saying these words, a good example is Rubens Barrichello who used to drive for the Ferrari team in Formula 1. In almost all his interviews, these words will be spoken - I don't know if he knows what he's saying but I think, it's a habit for him.

Yes, these words, if taken literally will mean exactly that!

I have found it usually means that whatever they're saying NEXT is the lie. Lies seem to always follow "To be honest..", or "Quite honestly..." I don't know why, but I'm always leary about anything I hear after something like that.

I think it is a combination: they are about to be a little more open than usual + they believe they are right concerning the topic.
