there isn't a good equivilent. "tomboy" is an outdated term that means roughly "girls who like sports and bugs better than dolls and lipstick." or, girls who don't conform to the narrow standards that women are expected to fit. these standards still exist, but at least today it's more normal to not fit them.
possible equivilents that don't work:
gay/queer/fruity: sexual preference has absolutely nothing to do with having effiminate personality traits/preferences. sexual preference and effiminateness are not mutually exclusive, and in my opinion, are only related at all as a result of our culture.
transexual or crossdresser: this is a whole different thing, much "stronger" words. tomboy is liking "tougher" activities, nothing more. A crossdresser or transexual is someone who identifies with the opposite sex/gender or someone who is born with genetically ambiguous genitalia and related characteristics.
metrosexual: this word is about adults, while tomboy is more often used with children. metrosexual is caring about one's appearance, which isn't necessarily a "girly" interest. especially when a person is "metrosexual" because it will "get them laid" or because it is helpful for fitting in with certain types of work environments.
effiminate. i think this word works best. however, it's more about personality and less directly about behavior. it's also an actual word, not slang, and doesn't have a negative connotation. (except, when used in certain contexts.)

A lot of people would call him gay or something similar, but I figure one ought just to call him a guy. Gender bias is really old-fashioned.
