1: Commas on dates always go after the number and before the year, never the month.

2: This sentence is inverted. Really, it doesn't make a lot of sense, but that is where the comma goes, yes.

3: In this case, the words captain and crew are not naming the captain and crew. They are improper nouns because they remain undefined. Do capitalize Voyager, though, becuase it is a name.
