I'd have to say the Viscera Drop by Viscera. There can't be that many things more painful than having 500 lbs landing on top of you.

any finisher tat utilise "Gravity" is painful....all the slam, piledriver kind of finisher, are actually "cushion" in a way to lessen tha pain.
submission finisher can be equally painful too but tat will be subjective depending if the wrestler applies any pressure on it or just simply holding on....

I think any body using a DDT finnishing move would really really hert in real life. I mean you can seriously break someones neck. Also The RKO or edges spear, in a real fight those moves are possible to do.
