It's a put-down, literally. They mean you'd have to do something really demeaning, like kiss their a**, before they'll do or give whatever they think you want. Presumably you have too much self-respect so you're not getting whatever it is.

"Suck my d*ck" (or some variant thereof) is a stronger version of the same general concept.
