She's very lucky. Some people can eat and eat and not gain an ounce. When I was 30, I had 3 kids and was still wearing a size 5. Then, when I hit 35, my metabolism started to slow down and I have not seen that size 5 since. Your friend should enjoy it while she can.

She must go without eating often to be extremely skinny. She might eat in front of you because food is available. Sometime people like this force them self to throw up the food after they consume it to stay skinny and have confused identity about their bodies. They are-anorexic or bulimic. they can have a fast metabolism, but more often then not they have an eating disorder if they are extremely thin or are starving for whatever the reason might be. Be it lack of food available or forced starvation. Emotions can play apart in these diseases. Encourage your friend to see a doctor.

If she is malnourished then she could have a problem, but if she's thin but eats a lot, there's nothing necessarily wrong with her, she just has a high metabolism. This is partly genetic, but also heavily based on hormone levels such as TSH (thyroid stimulating hormone) and various other hormone levels. Imbalances of testosterone and estrogen can affect weight. Anyway, basically if you increase your physical activity, you're metabolism will also increase.

No, it's not genetic! Nothing is wrong with her! I eat a lot and I'm still skinny!

Has she always been thin? It could be quite normal or she very well could have a thyroid condition, Graves.
