Depends upon the state, but usually the plate transfers like a vanity plate from car to car but stays with the owner. If not, the owner is issued a new handicapped plate. But more and more states are going from the plates to the window hangers so it would be a moot point in some areas (as the window hangers/paper plates certifying disablity) transfer with the owner of the tag

For my state (and I have Disabled Veteran's Tags), the plates stay with the Disabled Driver -- and transfer to the new vehicle. Some states have different rules for their vehicle tags -- so check with your state's vehicle tax office before you trade in your vehicle. Also -- your dealer should be able to help you out with this question as well.
