Take the divorce decree down to the Social Security office and the Drivers license place and they will change it back for you, pretty much like you changed it when you got married.

You can file the paperwork for a legal name change. You don't need to do it as part of a divorce, or have the divorce amended. It would have been "cheaper" to do it at that time, but if you didn't there is a separate process to legally change your name back.
You may not HAVE to do it through the courts, if you aren't changing your name in an attempt to defraud or evade payment of outstanding debt. You need to research what your state requires, and go from there.

I assume that you mean when you get a divorce.
Have your attorney list in your papers that you are requesting your maiden name back and the judge will make it legal for you to do so when your divorce is final. Then you just take your divorce decree to the Social Security Office and they will issue you a new card, then you can go to Banks, License Bureau, etc, to get everything changed over. All in all it takes about 6wks after your divorce is final.

If you mean you are wanting to go back to your maiden name from a married name - you always have your maiden name - it is on your birth certificate after all.
You change your driver's licence first (or other main form of ID) and usually just produce the divorce decree together with your birth certificate.

if u r getting a divorce you can tell your lawyer 2 add it into the paperwork. or u can go 2 a lawyer 2 get the change done. dont forget once you do u need 2 get a new ss card, licence and all other id must be changed. credit cards banking everything must be done one at a time. its time consuming but if u change ur name n dont do it u can get into all kinds of legal problems.
