I think he is in pretty bad shape. He didn't just hand the controls to his brother during surgery, he made long range organizational preparations.
I don't think C. Rice would be publicly going to the media and assuring Cuban people via the press that the United States respects the sovereignity of Cuba if our government didn't think some fairly large change was in the works.

Castro has been laying dormant for too long. If he does not return to power I think Cuba will remain the same.
