You want to create a non-profit corporation. Nolo Press produces good books on the subject. I have the California one, but there is also one for other states as well. The one thing to remember is that you need at least two people, so that the President and the Treasurer are not the same person.
If you have the money yourself, and want to make it a privately funded trust, an estate planning attorney can set it up for you. That does not require more than one person, although it's a good idea to have a successor trustee named to take over in case you are incapacitated or when you die. Once you have the funded trust, you can hire an executive director to do the actual work, and do as much of it as you want. For example, the executive director may scout out buildings or property and architects if you prefer to build from the ground up, but leave you the final choice in what the place will be like. Remember that if you buy an older building, it will probably have to be retrofitted for handicapped access.
I met a guy once who was successor trustee to his mother in just such a place. She was handicapped herself, and wanted a place she could direct and be involved. They bought and renovated an old church in the east bay (San Francisco Bay), and the main part of the church was the "great hall," just like in medieval castles. It was a wonderful place!

Your local health department should be able to help. Also try Better Business in your area. You can even call and ask to have a sit down meeting with a nursing home to see if they have any inside tips.

You need a lot of money and volunteers!!! If they will be charged to stay there then you could get a loan from the bank to start up..but if you are making it at no cost to them you need to have a lot of walk-a-thons and bake sales...etc Good luck to you and may God bless your kindness
