it means your not wearing any underwear, Im guessing since commando's are an elite force in the british army, that is has something to do with them

It came from "Friends." Joey put on a bunch of Chandler's clothes (several layers of shirts and pants)... to top it off, he "went commando in another guys fatigues." He then proceeded to do lunges.
