Generally not, it will run through the final installation of your OS and might request key codes for the OS and Office.. It depends on the vendor.
As someone mentioned you will need to setup your connections to email and your ISP.
Dell also does updating to drivers and oftens sends along reference CD's.. Do the updates..

Naturally, you will want to remove it from the box, and packing material. You will have to install the battery, but that is easy. Plug in the power cord, so that starts charging things.
Then you want to connect your High Speed internet (Ethernet Cable) or telephone line. The first thing that happens is the on-line registration thing.
Turn the computer on, and go through the complete registration thing. Then it will try to connect to the internet and submit the registration. If this fails, you can use SKIP and do it later.
Test everything. If you have a good internet connection, you want to use the Windows Updates first. Then go into your Anti-virus software and register that. Then get the lastest Anti-virus updates.
But YES, it's ready for you to use RIGHT AFTER you register and update.

If it's a mac then yes. Otherwise for windows I think that you may have to download some software but I'm not sure can you give us more info on what kind it is and where you got it from?
EDIT:
Ya if it's a dell you have to run some software so it'll take anywhere from @ 10 min to install everything and if you bought additional software then you have to install that too.

Yes, if you mean with it plugged in. The battery is most likely not charged. Otherwise yes. There is probably a setup wizard that takes maybe 5 minutes to do, but other than that yes, right out of the box.
