I got it immediately, and my IQ is enough for me to know I should not brag about it.....
Possibly because they are used to bar jokes, so the front part is something that they don't think about, taken for granted if you will.
Then when they realise the joke is ended, they have to go back through the joke.
Edit: I will admit that it took me ages to get it the first time I heard it, but luckily I have a freakish memory. :)

It's easy to get the joke, but it's hard to laugh at it because it's not very funny. It's for children.
It's a play on words, and that hangs some people up.
