I am looking for what exactly "proof of dependency" is, and how do I go about finding this form. My common law spouse needs a "proof of dependency" to claim my biological daughter for legal purposes.

Proof of dependency doesn't come in a form. You have to prove that you are supporting your daughter in order to claim her as a dependent. This could be medical bills, school report cards with your address on them, and ID, or a notarized letter from the biological mother stating that the daughter is residing with you.
