File a petition in the court where you reside. Your request for change of name will be published in a newspaper of general circulation. Afterwhich, the court will hear your case and decide if your request will be granted.

The exact procedures will vary by state. Here in California, you have to file a petition with the court and publish a notice in the newspaper for a certain period of time. The court conducts a hearing on the reasons you want to change your name. Unless there is any opposition, the court will usually grant the request and give you an order that you can take to the various government agencies to get your named changed in government records.

If you live in the U.S., it's actually very easy. You just start calling yourself a certain name and using it all the time. If you have no fraudulent intent, you are allowed to do this.
However, since 9/11, you won't be able to get any docs, like driver's license or SSN, without a court order changing your name. In the U.S., state law determines the procedure and court used.

I'm not sure of the exact procedure, but you can get it changed if you go to court and file some kind of petition, or form. Contact your secretary of state or attourney general for something like that. Or, post your question somewhere else in Yahoo! Answers.

You can. You have to go through the courts but you have to have a valid reason for it to be approved.
