A leak or crack in the radiator or reservoir?

Well, if everything is working properly it could just be your thermostat. But it could also be a leaky hose (connecting to the radiator), or a crack in the radiator. And if you continue to run it hot, you might blow the head gasket which is a pretty expensive part. My suggestion is get it to a mechanic quick.

There is really no way a exact diagonsis can be answered on here. It could be a number of things related to the cooling system. Take it to a reputable mechanic and have him do a cooling system check.

some late model cars have a small bleeder *(similar to a brake bleeder) valve to bleed air from the cooling system. if the car has one and you have had to add coolant you could have an air pocket in the system,( not allowing system to completely fill with coolant). any reputable mechanic should be able to tell if your car has this with just a look and no tear down.

You could have a stuck thermostat which is the cheapest to fix, a leaking radiator, a collapsed hose or hole in it and as bad as I hate to say it you could have a blown head gasket.
