Normally you put the mothers name on the invitation. Unless you are like me and had a co-ed baby shower for me and daddy. In that case my mother in law put both mommy and daddys' name on the invitation.
If you have already decided on a name you can put that on there either alone or with you or you and daddy or just baby.
For me, Although I put me and daddys' name on the baby shower invitation. I already knew what my sons' name was going to be but I didn't want to tell anyone. I wanted it to be a surprise.

If it is a female only shower I would say just the mother's name. If it is a couples shower then place both. If you know the sex of the baby it is nice to add that, otherwise I wouldn't put the baby on the invitation.

When the shower is being given just for the mother, then you would just put the mother's name. If you already know the baby's name then you can include that, too. Example:
"You are invited to a shower for MARY JONES and baby ANNE on August......." or if the baby is born:
"JOHN AND MARY JONES invite you to a shower to celebrate the birth of_____"

All three if you'd like. But you usually put the mother and/or father's names.
For example:
Please join us for a shower for:
Amanda, Jason and baby Evelyn
(or leave out any name you choose...it's really up to you)

Mom's or Mom and Dad depending on wether it's a co ed party or not...but definately not the baby's name unless he or she will actually be present at the shower...(if the baby's not born ..no)

My invitations had mine and my husband's name b/c my son wasn't born yet and we didn't know what we were going to name him. It just said and baby Boy or baby (last name).

Is the baby born already? If so I would say the baby's name. If the baby hasn't arrived you could say in honor of So and So's new baby.

My invitations had all three. It helped with personalizing gifts and cards. BUT beware: if the name is tentative and people get stuff engraved or embroidered, then you change it, that wouldnt be too good.
