Most of the above answers are incorrect. A shemale is not one half of a homosexual couple. A shemale is not a person who is transgendered.
A shemale is a male who takes female hormones to grow breasts. Men do not need surgery to grow breasts as they will grow on their own under the influence of female hormones. This can be confused with transgendered men because they also take female hormones to grow breasts.
However, a shemale male may or may not be planning to have transgender surgery so it is not defined by that. This male may or may not be homosexual or bi-sexual so, again, it is not defined by that. They are called shemales because outwardly they appear to be female and they do have real breasts. Often the illusion can last right down to having nothing on but panties. However, they still have the full male genitalia. This makes them appear to be sort of halfway between male and female, hence the name, shemale. This should not be confused with people who are born with indistinct genitalia or genitalia of both types (hermaphrodism).

The only person I know of who call him/herself a "shemale" happens to be a transgendered person, born male and with a penis but growing breasts due to hormone treatment. But whether this is an example of what the term "officially" refers to, I dono.

it's not a transvestite, it's a transexual (second language helps diferentiate between the two - vest comes from the Latin or Spanish "Vestir" meaning dress or to dress)
A shemale is a man who has undergone surgery to have breasts implanted on their breast plate but did not undergo the surgery to remove the penis. So they have both male and female body parts (breasts and penis)
And anyone who answers you with something that is not similar to what I just posted is DEAD WRONG. Get your facts straight and stop confusing this poor child...

My description would be
shemale, ladyboy - spoken term used in erotic industry. Mostly they mark transsexuals without main sex-change operation (pre-op TS).
transgender - term identifying / concerning crossdressers, transvestites and transsexuals. Trans means in Latin “through” or "cross", so freely translating it's like "changing lanes
