I usually go with "Congratulations." Then I tell them how cute their baby is -- all the time wondering if it will grow out of Winston Churchill's face.

first of all try NOT TO LOOK DISGUSTED focus on the mom's FACE tell her "good job" "did great" and focus on the baby, tell her he/she looks beautiful.
