well, since this question was posted in "Dining Out" I would suggest Olive Garden for a date.
It depends though, if its a first date and you really dont know the person very well and arent comfortable with them yet, eating out in that situation, you could do something fun like Dave and Busters or even Chuck E Cheese...may sound silly , but think about it or go to a place that has big TV screens.
If you are comfortable with the person and want a really good date, go to a place for conversation, like Olive Garden. You can even get takeout and go to the beach!

That depends, what does he like to do? Is this a first date?
I know a place that has amazing buffalo wings and music, the service sucks and the pool tables are expensive but the food and drinks are worth it. ... also there is an amusement park nearby if he likes that sort of thing

Take charge and make plans. I would take her out to dinner, and ask her what she'd like to do once dinner is over. If she doesn't come up with anything, suggest taking a walk, or getting drinks...If you asked her out, you should make the plans.

somewhere different... where you can talk, but not be all alone, that shows that you put thought into it, but you arent creepy, and shows an interest in her.... IE... if she likes animals go to the zoo or an aquarium... if she is into sports... get GREAT seats at her fav teams next event... if she like music or musicals, plays are REALLY fun... if she is really classy take her dancing ( not clubbing... Or out to dinner.

Or to the park.
Or the mall.
Or to a movie.
Or rent movies and watch them at your house.
Or just for a walk.
Or you could go to an amusement/water park.
