Bleeding and cramping.
If a woman experiences spotting (bleeding) it can be a sign of a miscarriage however in early pregnancy spotting can result from the egg implanting in the lining of the uterus.
A pregnant woman should go to the hospital or her ob/gyn if she has bleeding or severe cramping.

There aren't always signs. I lost a fetus in my 5th month. The only way I found out was at a prenatal exam when the doctor could find no heartbeat.
An ultrasound confirmed the baby had died.
Those other answers, though, are true.
