I know that tall people like them mounted higher, but what is the height that most plumbing contractors mount them in a new house.

6 feet is only a minimum requirement in most code books. You can set it higher for you personal preference. The guy that is 6'4" is going to have to duck down if it is mounted too low. I have run into this problem often and my advice is threefold: move the plumbing up,get a massage type with a hose and mount the bracket high enough to compensate, or get the bar that extends straight out that is adjustable up to 2 feet that has a short 90 at the end for the shower head.

The pipe usually comes out of the wall at 6 feet 6 inches from the base of the shower stall. This is VERY annoying for me because I'm 6 feet 4 inches tall and end up having to duck in order to get under the shower head. Bear in mind that the pipe that comes out of the wall usually has a 45-degree bend near the end of it, so the shower head will actually be lower than 6 feet 6 inches.

It is your choice. I just had my house plumbed and I have (1) at 6'6" in the guest bathroom. In the master, I have two shower heads, one at 84" and another at 6'6" that I will be using with an adjustable "arm/hose" so that I can choose different heights or pull it off the wall and use it as a handheld sprayer. I am 6'4" tall and I have to duck underneath most shower heads to take a shower. You can't really go wrong by putting it a little bit higher than normal

I agree that 6 feet above the ground sounds like the industry standard, and for taller folks, mounting the shower head higher would be much more comfortable for them, I know my shares of gents who are 6'2" or taller.

72"

Some people go higher or lower in DIY projects based on their desires.. I have a son that is 6'4" I'd place the shower at about 6'4"-6'8" to accomodate his height.

Being over in Germany right now, I prefer the vertical slide bar with the moveable hand wand. Since I'm 6'3" and my wife 5', it makes things much easier that way, and she really, really enjoys the pulsating massage spray head.

A shower with the slid bar seems to be the best.
