You must mean a "Healthcare Power of Attorney" or a "Living Will." Is the man dying? The hospital should have those on file, the "basic" forms you need.
Besides, asking for an attorney on Yahoo Answers is like asking calling out on the speaker phone at Walmart and asking if there are any plastic surgeons available.

and put your name adress and number. they will call you and then
