If I have understood this question...you are wanting to evaluate your lessons with special needs pupils???
What I use is three faces.....one sad...one normal...one happy....and ask them to point to the face they feel...when I ask them a question. Questions asked are:-
1. Have you learnt anything?
2. Did you enjoy?
3. Will this be of help to you in the future.

If you are looking for a comprehensive evaluation of a child's abilities... there are two I can think of. The first is used by preschools and schools in California- The DRDP (Desired Results Developmental Profile) which lists lots of areas of evaluation (social, gross motor, self-help skills, cognitive, etc.) If you are looking for a tool for a closer, more in-depth look at a child's specific abilities, you will want the HELP (Hawaii Early Learning Profile) Strands. This tool breaks it down by month what you can expect from a child at their age level, and gives an accurate picture of how delayed a child is in which areas. You can download each of these from the web.

Do you mean that you are looking for tests (evaluations) to administer?
If so, you shouldn't be doing so. You need to be properly trained in the administration & interpretation of these assessments.
Let the professionals complete the evaluations...Contact the child study team and the Learning Consultant or the Psychologist will do them for you

Create you own. That is pretty much what I do as all the tests and formuli that I have seen being used miss the essential "me" inside these children. As their carer you are the one best able to bring it out through love and respect.
I do use Gars ratings and such like to keep the parents happy but they are mostly cosmetic.
