Very quiet, soothing, caring. Sometimes I hear it in English, my native tongue. Sometimes it is a thought or mood. Regardless of the method of delivery, I always recognize it.
I also know from experience that I often hear it when I am about to do exactly the opposite of what I should do. For some reason I don't always listen, even though it's been giving reliable advice for 55 years.
My personal belief is that this "inner voice" is actually an internal thought generated by an under-used part of our minds. Other folks believe it to be the voice of God, a "guardian angel", or some other spiritual being.
Whichever version is true doesn't matter. What matters is that not enough people in the world seem to be taking the advice. Soldiers fight against it (and indeed are trained to!) when they go into battle. Others ignore it when it says "You have to leave" or "THAT'S THE ONE !!! SAY SOMETHING !!!". For drunks like me, it's the quiet scream "no" when we're about to take that first drink.
I think the Buddhists are on to something. The goal of their type of meditation (if "goal" is the appropriate word - any Bhuddists out there have a take on this?) is to dispense of all thoughts, right down to the conscious self. What's left is supposed to be the sensation of being "in the now", "in the present moment", or "being connected to all things". Personally, I think that once the self is quieted down, that "inner voice" will be what remains. The "connection to all things" is the connection to that voice, the "eyes" of which can see a lot more than our conscious selves.
Alcoholics Anonymous folks call this "The Higher Power" and sometimes equate it with God. I like the "inner voice" better, because I believe that it's a capability of the human species which has evolved over time, and which is now becoming more and more present in our minds. The people who survive well are the ones who use this mind. That's evolution in action.
Whatever. It's there, it works, that's all that matters. Too bad not enough of us, religious folks or evolutionists, listen when it says "never mind what I am; get along, you're all nice people."

Mine is me!!!
And the tongue depends on the situation and what i am reading/watching at the time. Jane Austen is my favourite but it can go Bridget Jones where you leave out the "I" in every sentence and start everything with "Am".
Or Oscar Wilde if I am feeling contrary.
