If it has been 5 years and your wife has not addressed this issue. Not even from the side, it is very possible that she doesn't feel strongly enough about her needs to risk rocking the boat. You, my friend are going to have to convince her that her needs are as important to your relationship as any other member of your family. And that really is the truth.
Women can get involved in their families lives and be totally content wrapping their life up in those that they love. Especially where children are involved. There is one need, however, that a woman absolutely cannot function properly without... The need for security.
This is the most important of your husbandly duties.She is a woman and she is going to worry but you can have a large impact on how much and to what depths these worries occupy her. First and formost, she should never be given reason to doubt your love. You need to take every opportunity to tell her you love her. Look her straight in the eyes and make her hear the words.
Your actions must be purposely geared to showing her that you find her beautiful and desireable. She needs to believe that, where you are concerned, she has no competition. She needs to be held close. She needs to feel your hands on her body. She needs to see your concern for her happiness. She needs to step away with you into a world that frees her body and mind to explore the wonders of your combined worlds as well as time away from distractions to experience what she is as a person, a wife, a mother, a sister, a friend.
She may think it is selfish to devote energies to her own needs when she see so many needs around her that are unmet. She needs to know that it is not selfish to have desires and goals that aren't neccessarily the same as yours. If man were meant to possess all of the intricate pieces to this puzzle we call life, there would have been no reason for women to have developed an equal mental state to that of their male counterparts. The two shall be come as one. Not as one or the other. She needs to bring her unique personality, gifts, etc... into your relationship to insure the next generation has a fighting chance.
I believe with all my heart, if you can bring her to this point, the rest will come easy.
