However, there is some little consideration to do.
A friend of mine often noticed I had just got a handjob. In fact, after any sexual act, our eyes are wet and sparkling! Generally, it needs few times to re-establish the normality.
In some cases of masturbation addiction, the blood exam can reveal that long habit.

No there is absolutely no way to tell if a boy masturbates or has had sex. Even if you did it an hour before you see the doctor.

No your doctor cannot tell if you masturbate or not.
And he does not care.
However, at some point, he will assume that you do, as all people do. It is normal and healthy. Don’t worry about it.
I have never been asked about it in an exam.
