You have plenty of time to get a US passport.
The link: http://travel.state.gov/passport/passport_1738.html
will assist you in getting the proper form.
If you have never been to Africa before, it will be a very unique experience; Lagos is a great place to visit; however be aware of pickpockets; learn to bargain whenever you buy something, never give what they ask, start at about 1/3 of the asking price and increase in small increments until both agree on a fair price.
The best connection, in my opinion, is Washington DC (Dulles) to Frankfurt, Germany and on to Lagos

Don't forget to get immunization for yellow fever and take malaria prophylaxis, starting at least two weeks before travel.

Good luck!
