get her something you know she really likes or needs.. if you are not sure ask her about what she likes but don't make yourself obvious if it's a surprise

Something that will mean a lot to her. A bracelet, necklace, something that shows her she means the world to you. By the way, it does not matter if it is a 2 dollar gift if that is all you can do. Try putting a letter in the gift to and I'll bet 10 years from now she'll bring out that letter when you are together chatting about growing up.

First, know more about her. Try to know what she likes and desires, and perhaps, her own style. Don't just go up to her and ask,"what is your favorite color?" or "Do yo want a purple dress?" If you want to surprise your sister with your present, then stop asking questions about what ou will gve her. You an observe her throughout the days, and listen, pay attention to whatever she talks about her birthday. She will eventually give you hints what she want.Once you think yo know what she wants, go for it, and good luck!

Make her a gift basket with her favorite sweets and some nice smelling lotion and shower gel and candles girls always like that stuff. Make the basket look all pretty.
