SOAP- subjective, objective,assessement, and plan. That's hospital, in a Clinic it's not so formal. You can read your chart if you want!

Nurses' notes are not a big mystery. Ask for a copy of chart notes and have someone that knows med terminology to sit down with you. I can tell you as a Medical Assistant, the one thing that should NEVER be in them are personal thoughts and ideas. Like the Paramedic and Nursing Student said what the nurse sees, feels, observes, is told by the Dr or you and other important info is included.

Nurses notes only say what care was given to the patient, what issues were discussed that effect patient care, and how the patient is responding to treatment and care. Interventions, reactions, improvement, ect.

I am a retired nurse and when a nurse charts, she uses a lot of abbrev. I did some charting one time and after I read it I had to laugh. If you refer to right or left eye-arm leg or anything, then you would either put an r or an l with a circle around it to indicate right or left. My patient had complained of pain in right ear and so I charted -patient complained of pain in r ear

Yes. They say what treatment the patient has received from that Nurse, her observations and the patient's vital signs. Why?
