It depends on the laws in your state, or, under federal law, it depends on what type of work you do and how you get paid for that type of work.
Generally, if you are an hourly employee, you are entitled to two fifteen minute breaks, one two hours into your shift and one two hours before the end of your shift and a half-hour, unpaid lunch, if you work an eight hour shift (i.e. your shift is from 9 to 5:30 with 30 minutes off for lunch equals eight hours on the clock).
These breaks are not guaranteed and if you are not getting your entitled breaks, you may be entitled to some other compensation, like over-time pay.

Visit your state's Labor and Workforce website. Different states have different laws that specify, but generally, if you work X number of hours, you are required to have a break. If you work X number of hours, you are required a lunch break. There are different types of jobs, (like salary positions) where your employer isn't required to schedule or force you to take breaks. It is left up to you.

If you are under 18 and working at leat 6 to 8 hours, yes I believe you are. If you are over 18, you do not have to take a break if you don't want to.

Depends where you live- you'd have to check local regs to find out. In California you would be - can't say what the story is in other states. Call your state's attorney general's office - they usually have someone available to answer consumer questions.
