NY islanders.
It has a map of Long Island on their jersey.

Los Angeles, Buffalo, Tampa Bay, Anaheim, Dallas, Toronto, Colorado.
Buffalo and Colorado have the city names on their 3rd jerseys. But the Sabres announced a recent change in their uniforms. Los Angeles is across the bottom of their jersey, Tampa Bay, Dallas, Toronto, and Anaheim all are part of the main logo.
I don't know why the question states that there is only one team unless it is a technical question.

The Tampa Bay Lightning have Floroida on their shoulder
The Stars have texas as well on their shoulder
The Capitals have the Capitol building on it.

I thik it is the Buffalo Sabres. Their logo is a Buffalo head, not Sabres. In the old days, the logo had two crossed sabres over a Buffalo.

I know Tampa Bay does. They have a picture of Florida on thier left shoulder.
