Ok a performing resume is different from a job resume. You want to be sure to include your height, weight, vocal range, training etc.
Take a look at this one. She's a friend of mine and a fantastic singer. While it is on a web page, her resume looks exactly the same on paper.

we handed copies of those books to our graduating class (a dance school) and I kept a copy for myself.
