No, most businesses are not required to provide a shower for sanitary purposes. Some businesses are required under OSHA to have showers due to chemical contaminates that must be removed or to have the shower present incase of some type of emergency. It is a courtesy that businesses have locker rooms and/or showers for their employees.

If you're talking about the actual instillation of facilities - it's a good idea. Not only is it useful to people who bike commute but to your employees who duck out to exercise at lunch and a healthier employee is a happier employee and a happier employee is a more productive employee. Also, you can often write off instillation so check with your tax accountant for more information.

It depends on how many employees work out before and after work and how much you wish to do to keep them happy. It is a nice to have, and many employees do appreciate it. Many employees will bicycle to work if paths are available and there's a shower at work. A heathier workforce is also good for business, so working out should be encouraged.

That would probably depend greatly on what kind of workplace you are talking about. If it is a place where workers get extremely dirty, it would be a good idea.
