I can't understand why your GYN wouldn't suggest seeing a urologist if the GYN doesn't want to do it themself. Insurance should cover it. Although, castration might not completely remedy your situation. You still might need hormone pills. Make sure that your ovaries and adrenals are in tip top shape first.

Good luck

Is a ts girl a guy in denial? Oh wait I know. T s means Transexual
I'm a vet. Two years in Nam. I've castrated a few VC so I'd be willing to castrate you for a fee. It's not painless so You'll have to bring your own anesthetic if you want some.

But seriously I know a person who had it done. You should find other ts girls to ask about this. There are doctors who will do it. They usually make you go through psychiatric evaluation first.
There should be a website somewhere for trannies where you can get advice and support. Good luck. I understand how you feel. I'll never understand why.

You could consider going to Thailand, they are really good at it, and many people round the world go there to get it done. They actually offer packages which include everything, not just the operation but also travel, hotel...

And if you go to Thailand, you are bound to see a few people and wonder whether they are men or women; many of the transsexuals in Thailand look much better than regular women.

Do a lot of research and talk to as many people as you can.

I can point you to a few websites I found, but since I did not go through such an operation, I cannot personally recommend any:
http://www.pai.co.th/
which includes the doctor featured in:
http://ai.eecs.umich.edu/people/conway/TS/SRS/Dr.Preecha.html
Another couple of websites:
http://www.bangkokhospital.com/sex-change.html
http://www.chet-plasticsurgery.com/

Good luck and be happy!
