They usually just make you strip to your underwear...and just take those down a bit to check your penis and testicles...and to make sure you don't have a hernia....and if you are over 40 you probably will have a digital exam of your prostate gland....( a little uncomfortable, but I'd rather make sure every thing was okay)

It depends on the type of appointment you have whether or not you have to remove any clothing or not. Generally speaking the only times you would have to take off clothes are for appointments about a part of you body that is normally covered up. For example, a gynecology appointment. It is not until toward the end of the visit after you have gotten a chance to voice your reasons for being there and your concerns. Just make sure you have a doctor you trust and are comfortable with...they are just doing their job.
Obviously I am not going to know first-hand what happens during a doctor's visit for a guy, but I would imagine they wait until the end of the appointment after talking about the procedure.

Oh yeah,and put on the old hospital gown. Oh, and bend over to get the old prostrate checked. Have fun.
