I'd vote for icebreaker.

A casual "How are you doing?" in our society is about the same as "Hi."

Voice infection might change how the question is interpreted. If you say, "How are you *doing*?" you might get more of a response than "Fine, thanks. And you?"

I think it's mainly an icebreaker. People usually don't want to hear how you are doing, I don't think they even really care but it's a way to get the conversation started. ^.~
