Well for starters, just wash your back...I would assume you shower...and most often the last thing you do is rinse the conditioner out of your hair, assuming you use it, if that's true, be sure to at least rinse your back clean of any excess conditioner, it WILL clog pores! Secondly, If you use hair spray or gel in your hair, and it gets on your back, it will also clog, so be sure to wash after spraying and yada yada yada, you get the point? :-D I used to have a slight problem with it, now I don't, and all I do is this. Use Alpha Hydrox Enhanced Lotion and benzoyl peroxide.
1. Wash the area gently with a mild cleanser and let dry.
2. Apply the alpha hydroxy lotion and wait for it to dry.
3. Apply moisturizer if needed.
4. Apply benzoyl peroxide generously. (You can flip-flop step 2 and 3 if you'd like)
TIP: Benzoyl peroxide bleaches fabric. Wear something you don't want bleached.

Depending on how serious it is? Tetracycline Hydrochloride 250mgs 4 times daily for 2 weeks. This is a prescription drug so see the Doc

I have teenaged daughters who use "phisoderm acne skin care body wash" and a body sponge on a long handle so they can reach all of the back. Works like a charm.

My sister used to have it really bad a few years ago, and i know it seems strange but my mum but methylated spirits on her back every single night for a few months - it cleared up perfectly and she hasnt had one zit since.

Ummm maybe you should use a back scrubber and just use some soap. I have also heard proactive works really well. They even sell pads for your body.
