The vaginal canal is stretched quite a bit during pregnancy, if a woman doesn't take care to do her Kegel's exercises, then her vagina will remain stretched out and loose. After several children, exercises can only do so much, there are surgeries availble to take up the loose tissue and snug things up.

Episiotomies (which can leave some scar tissue at the mouth of the vagina) can give a vagina that appearance. Usually takes more than 2 or 3 of them though.

no, it takes some time, but usually after 6 weeks you're ready for normal sexual activity

No it doesn't, it goes back to the normal size, or so I've read.
