It hurts. They don't actually turn blue. It just hurts a lot like they're being squeezed or crushed. It's technically called prostatic congestion and is caused from too much arousement. Just lay down, relax and put some ice if you need to. DO NOT attempt to orgasm, it will make it worse.

How do you know? Check and see if there is a woman within arms length of you. If so she's probably partially -- but not fully -- undressed, right? Or is she getting dressed now? Is she all kind of huffy, stomping around? Has she thrown anything at you -- yet? Are flames shooting out of your eyes?

Or is she saying things like, "sorry, gotta run, I'm having dinner with the girls, blah blah blah"? Is she completely cool, calm, collected, sweat-free, make-up not even smudged, happy as a lark, entirely oblivious to you?

And you: are you fully undressed (I'll wait while you check it out)? Do you have an erection that is getting softer by the second? Is sweat pouring out of you? Are you hearing yourself offer the huffy, now fully dressed woman things that you don't even own nor could ever hope to possess?

If these things are true, you, sir, have blue balls.

They call it blue balls because it sometimes causes your "balls" to become sore. It is basically a tension created by not being able to ejaculate, therefore making some parts sore, or "blue" like referring to having them bruised. Most guys don't get blue balls until after a few times of not being able to finish. The cure us to have an orgasm.

I have had em and it aint funny. It hurts! This comes from being teased excessively and not pleased if you know what I mean. I dont care what anyone else says a sure fire way to get rid of it if she won't is to jerk off--maybe twice!!

It hurts really bad, each step feels like you have been kicked in the groin, setting and even breathing hurts. It will go away in a couple of days if you are lucky. (soaking in a hot tub with Epsom salt helps)

You may have noticed a distinct bluish tinge to your testicles after you make out for several hours -- and the deep, most uncomfortable ache that goes along with it.

Actually, it's not your testicles that are blue, it's the skin of the scrotum. When you're aroused, blood flows not only to the penis, but to the entire area.

The longer you stay aroused, the longer the blood stays there. Newer blood is red, but older blood, which has less oxygen, is blue -- and that's why your testicles appear to contain more blue blood than Buckingham Palace.

It's not harmful, so don't run to the emergency room. If it lasts for hours and hours after you've stopped being aroused, though, see a doctor. Or find socks to match.


Blue balls is a slang term for a temporary fluid congestion in the testicles and prostate region caused by prolonged sexual arousal in the human male. It is most commonly associated with adolescents but can occur in any sexually mature male. It is often accompanied by a cramp-like ache of prostatic congestion and pain/tenderness of the testes.
Cause

The main cause of blue balls is prolonged sexual stimulation of the erect penis, either by direct or indirect contact, that does not result in orgasm and ejaculation.

During arousal in a sexually mature male, the sympathetic nervous system decreases its inputs to the genital tissues, resulting in increased blood flow to the testicles and prostate areas. As this happens, other fluid outflow muscles constrict, causing more bodily fluid to enter the area than leave, ensuring a high enough regional blood pressure to allow a sustained erection for penetration during sexual intercourse.

If orgasm is not achieved, blood and lymphatic fluid tend to pool, and the blood becomes oxygen deprived. In some men, this pooling of oxygen-starved blood may result in a bluish tint to the scrotum. It can be very painful, like a hit to the testicles except from the inside.

Men with priapism or orchalgia may experience an extreme, prolonged form of blue balls, which may require medical attention.

Treatment

The easiest way to relieve the symptoms of blue balls is through an orgasm either by masturbation or sexual intercourse. The resultant ejaculation jump-starts the sympathetic nervous system, which increases blood flow through the groin area, dissipating the fluid buildup. Even without orgasm, the symptoms of blue balls usually subside within an hour of onset, but the symptoms can also last much longer. A review of the medical literature by Chalett and Nerenberg in 2000 found little formal data regarding the condition but concluded that "the treatment is sexual release, or perhaps straining to move a very heavy object — in essence doing a Valsalva maneuver." Simply lying down can also sometimes help the pain associated with blue balls.

One folk remedy for blue balls is the cold shower. Putting cold substances (such as ice) on the crotch region significantly helps. One source [1] states that "the cool water of the shower would stimulate new warm blood to the scrotum," but local cooling might instead work by causing arterioles in the scrotal skin to constrict, thus decreasing blood flow to those tissues and allowing fluid to leave the congested areas. Also, physical exercise like walking, climbing stairs etc, can ease the engorgement. Sometimes drugs containing pseudoephedrine can help quicken the process; however, availability of medicine containing pseudoephedrine is limited in some countries. Analgesics do not generally help as they do not involve prostaglandins.

In women

Women can also experience discomfort due to unrelieved vasocongestion as their genitals also become engorged with blood during sexual arousal. They can experience pelvic heaviness and aching if they do not reach orgasm. The general term pelvic congestion refers to such pain as it occurs in either sex.

I think it's going to depend because the term gets used pretty loosely, sort of like "hooking up". It seems to mean different things to different people.
