How can this type of miscommunication be effectively addressed or completely avoided?

Miscommunication stems from inactive listening, interruption, and jumping to conclusion. I believe the list I have given needs self evaluation... unlearning what we've been accustomed to. Stop interrupting when somebody is speaking its totally rude and listen more, not just hear it... listen...

By then ideas can get across better...communication will be open

One source is different levels of vocabulary.

Solution- Make sure that you know the level of your "audience" and use vocabulary appropriate to the situation.
