There are a lot of variables involved such as the state you live and whether you are a minor or an adult. Where I live a petition of name change is filed in the district court of the jurisdiction were the petiononer resides. You will also probably need to change the name on your social security card and drivers license. If you are changing your last name due to marriage in my state you can do so by showing your marriage cerificate.

ask a lawyer,and cost is differ by private lawyer or public lawyer you are going to choose! To change name has a long process, you have to go to court for hearing and publish it in the news paper!Here in the philippines it cost around P10,000 to 20,000 pesos ($200-300).
