Since this is in "Comics and Animation," we won't bother talking about the movies.
Jean Grey has died three times. The first death occured at the end of Uncanny X-Men #100, where she died while piloting a space shuttle through re-entry. The intense heat of the shuttle's unshielded cockpit nearly vaporized her body. She was almost immediately revived by the Phoenix Force, which placed her body in a cocoon to heal for several years. Meanwhile the Phoenix Force used a copy of her body to fool everyone into thinking that Jean had been saved by the surfacing of her new powers. The Phoenix, still pretending to be Jean, died on the moon (arguably Jean's most famous death, but not actually Jean), and its conciousness was later added to Jean's, along with that of Madelyne Pryor, a clone of Jean who had also died.
Jean emerged from the cocoon and lived for years until being again killed by the sentient mold Sublime (posing as Magneto) in New X-Men #150. Sublime/Magneto, whose powers had been boosted by the drug Kick, sent a lethal pulse of electromagnetic energy into her brain.
Shortly after that, the alien Shi'ar attempted to finally destroy the cosmic Phoenix Force in the "Phoenix: Endsong" miniseries. Fearing for its life, the Phoenix returns to earth and resurrects Jean once again. After the threat of the Shi'ar is gone and both Jean and the Phoenix make some important realizations, Jean voluntarily ends her physical life yet again, moving on to another plane of reality.

Jean died when Magneto killed her at the end of the Planet X storyline. Nowadays, Scott has a relationship with Emma Frost. In the X movie, Wolverine killed Jean.

She hadn't died yet, but Professor X says she will turn into the Phoenix. Eventually the Phoenix turn into Dark Phoenix and is destroyed. Jean Grey always returns from the dead, though.

Well, in the comics, the 1st time she (sorta) died was when the X-Men were returning from a Sentinel base in outer space. There was huge sun flare activity (because the Phoenix entity was in the sun, as it turned out, but more later) and the shuttle was damaged so that its autopilot wouldn't work and the cargo hold was shielded but the cockpit wasn't - and anyone in the cockpit would die from the radiation from the solar flares. Jean read someone's mind to know how to pilot the shuttle, telepathically knocked Cyclops unconscious and sealed the rest of the X-Men in the cargo hold. She then flew the shuttle and tried to use her telekinesis to stop the radiation. The shuttle crashed in Jamaica Bay, the X-Men surfaced and then Jean Grey flew out in a new costume, calling herself Phoenix. (X-Men 100-101).
However, her powers kept growing until at the end of issue 134 her costume changed colour and she called herself Dark Phoenix, and quickly trashed the X-men. She then sent herself through a worm hole she created to Shi'ar space and sank into the star D'Bari, sending it supernova and destroying the 4th planet, which was inhabited. Then she destroyed a state-of-the-art Shi'ar Imperial Battle Cruiser. Then she returned to Earth where again she easily defeated the X-Men, but was stopped by Professer X who helped her to control herself again. But the Shi'ar weren't convinced, because Dark Phoenix fit their legends of the Chaos Bringer, a cosmic being more dangerous than Galactus. The X-Men had to fight the Shi'ar Imperial Guard on the Blue Area of the Moon, but were beaten badly. When Cyclops was knocked down, Jean Grey lost control, and burst into Phoenix again. This time the X-Men fought to control her and stopped her turning into Dark Phoenix too early. They couldn't stop it, and before she completely succumbed she used an ancient weapon to destroy herself completely. (X-Men 135-137)
Sometime later Cyclops met (and later married) a woman who was an exact look-alike, called Madelyne Prior, who turned out to be a physical clone of Jean Grey created by Mr Sinister in order to keep the Summers' genetic line going, but she was possessed by an evil spirit and became the Goblin Queen....
But before she did!!! The Avengers found a sort-of Pod thing in Jamaica Bay with Jean Grey in it. Remember that Phoenix Entity thing? It sensed Jean Grey in the shuttle, and tried to know/understand her better, but in doing so lost it's memory and thought it was Jean Grey. Subconciously it placed Jean in a Stasis Pod thing and took her place. Jean was revived by the Fantastic Four, and returned to the original X-Men team who formed X-Factor.
Since then, she's died again, but I don't know how. Also Dark Phoenix replayed itself several times.

Your all right if your talking about the cartoon.
In the comics though, she died differntly.
A mutant named Xorn was dressed like Magneto got the Xmen into space, and then used an electromagnetic pulse to kill Jean ... then Wolverine cut off his head. (New Xmen 150-151).

she dies in many different ways like getting caught in a sea of water at Alkali Lake. Wolverine stabs her. I believe she dies at the hands of magneto and Apocalypse. But she can always come back from the dead. Just like in Harry potter 2 and the Chamber of Secrets. Fawkes the Phoenix dies while Harry is in Dumbledores office but Fawkes comes back from his ashes.
P.S. I am so waiting to see a question on Harry Potter tee hee

In Uncanny X-Men, she commits suicide so she won't become Dark Phoenix and Destroy everything (literally!)
In the movies she first sacrifices herself to save the team, then she's ressurected as the Phoenix, then she's stabbed by wolverine. She's been ressurected alot of times, (she's Phoenix!) she's currently dead, (again!) but she'll be back, even on her grave it said, She will rise again.

Her alter-ego is Phoenix. Think about it. The Phoenix rises from the ashes. So can she really die? In the comic books she's died several times. She's even died twice already in the movies version.
