Terms such as shower pocket, douchebag, d-bag, and just plain douche are popular terms of insult or ridicule in the United States. It is often used to ridicule a person who acts as if he is better at something than he actually is. For example, a guy who dresses up like a basketball player, but is actually a horrible athlete would be more likely to be called a douchebag than a person who makes no pretense of being good at basketball, but is just as bad at sports.

I'm not sure you actually want to know and I hope my answer doesn't offend you. A douche is a flushing out of the vagina, a thourough clensing. A douche bag is what, I imagine, collects the waste. Therefore calling someone a douchebag is not a literal insult, but isn't a very nice thing to be called, and I imagine it would imply they are full of rubbish. Hope this helps
