you need to see your local citizens advice found in your yellow page or 118 118 just tell them whats happened take all medication also you can get clover leaf there people who can swing things for you there found in yellow pages 118 118 too

you mite not get it cos you havent paid enuf national ins.if you still give in sick notes you can get your ni contributions paid then when you have been off for 6 months you will then be elligable and they should send you a form automatically.i have been through the same thing.thay dont care if you are homeless and starving.i tried for 3 months.when i said how am i meant to afford to eat they just said theres nothing they can do.even my partners grandad who just had 2 strokes and broke his back falling off a bed in the hospital cant get any help.

i think you have to wait something like a year.
when you refill your forms put the info in using your worst days you dont get any rewards for trying to battle on just penalised.
