The really busy person is struggling, with veins popping out of their head, beads of sweat on their brow, and a serious stress problem.

Technically, someone who pretends to be busy, is busy, pretending to be busy! This makes it harder to differentiate. A busy person will be able to tell you in some detail what it is they are up to, whereas the fraudulent will attempt to blag their way through and be tripped up by questions requiring specific answers.

Well, difficult to say. But my experience says, a real busy person will listen to you if you can express your requirements in a brief. Those who are not actually very busy but just faking, won't listen to you at all.
