Someone who makes things up in there mind because they are not satisfied with real life achievements.
Usually tied to irrational depression, depression that makes you feel that you are meaningful, but have been cheated out of your destiny.
Is obsessed with something until it has been achieved, then becomes disenfrancised with it.
May believe that they know how someone elses mind works, and then imagines evil or good intentions from meaningless gestures.

Sounds like possibly narcissistic Personality Disorder.
Possibly touches of other stuff though, but if I had to fit all of that into ONE, I'd probably go with that.
