You have photoreceptors (rods and cones) that pick up wavelengths of light. When a bright light is directed at these photoreceptors they become "bleached" and it takes some time for them to full recover. This is why it appears there is a shadow -or spots - these photoreceptors are not functioning properly and therefore not picking up the light they normally would.

I use to see dots like that, I always thought it was due to blood pressure, or if you get hit in the head really hard you do see stars....

Its because your cones and rods get overstimulated, like a bleaching affect. Once they can replenish you can see again
