Probably used to be pierced there. I've never heard of any man having a second hole in his penis that was there at birth.

He is uncircumcised, therefore, he has extra skin covering his head...this serves as the second hole actually.
First the semen comes out of the hole underneath, and then through the second hole.
It's perfectly normal.
